Form Review
Campus Comment Period
The Performance Management Initiative, which includes the revision and consolidation of campus performance appraisal forms, is in its final phase. Following a year of committee research, development, and extensive consultation and feedback, the forms are now available for review and comment.
The new forms are located at:
Campus managers, supervisors and staff are encouraged to review these forms and direct their comments to Cecile Morris, Project Lead, (cecile@ucsc.edu or extension x9-3935) no later than 5:00 pm, May 7, 2007.
Once adopted by the campus, principal officers, managers or supervisors will select and use one of these forms beginning with the 2006-07 staff evaluation cycle (i.e., July 1, 2006 through June 30, 2007). The finalized forms will be enterable electronically and include comprehensive instructions.
