Performance Appraisal Forms
How to Use the Performance Appraisal Form
Principal Officers may decide which forms will be used within their divisions. Please check with your Division or Unit Head to ensure you are using the correct form.
- Select the appropriate form from the list below.
- Save the form to your desk top to complete as time allows.
- The forms are MS Word documents.
- Sections provided for entry may be expanded, retracted or, in some cases, deleted.
Page breaks can be adjusted.
- You will need to manually adjust the size of text boxes. You can do this by hovering your mouse over the text box frame until arrows appear. Then you may simply click and drag the box to the size you need to fit your text.
- You may copy and paste into each of the documents.

Performance Appraisal And Development Forms
- Option One is structured in a check-box format.
- Option Two reflects key functions from the job description, and previously set goals and objectives.
- Option Three is entirely narrative.
- Option Four allows for a review of essential functions and projects, significant performance dimensions, and goals accomplishment.
- Self Appraisal shall be used in conjunction with all four performance appraisal form options listed above. Each employee can print this form separately and complete it upon request from their supervisor.
