Performance Appraisal and Management

As part of the performance management process, UCSC has standardized the performance appraisal forms for use by all Managers and Supervisors who supervise staff employees. These instructions are designed to assist supervisors with navigating the new performance appraisal form options.

There are four form options that shall be used for the annual review and the probationary evaluation, along with an employee self appraisal form. Each of the form options include a section for assessing the performance of managers and supervisors in the areas of:

• principles of community
• diversity
• health and safety
• leadership

It is important for managers and supervisors to consult with their Division or Unit Head to ensure they are using the form(s) recommended by that Division.
