- Employee Disclosure Notice-Acknowledgement Form. (PDF)
- This form will need to be completed, signed by any employees providing covered services to UC, and returned to AgileOne.
- The attached Acknowledgement Letter (also available in Spanish, here) (PDF)
- This will need to be completed by any employee performing covered services at UCSC. All information pieces on page one need to be completed including the date and signature. Once contracted this form needs to be submitted to AgileOne immediately for labor contract and state law compliance.
