UCSC Family Member Eligibility Verification & Reverification Workshop

To make sure we use UC’s benefits resources wisely, periodically, UC will ask plan members to reverify the eligibility of dependents enrolled in Health and Welfare benefit plans. Some dependent relationships may change over time, and FMEV reverification ensures that only those family members who are eligible continue to be enrolled. Dependents included in the reverification may consist of your spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward or overaged disabled child.

Employees and retirees will receive a packet of materials from UnifyHR with detailed instructions on how to complete the reverification process. Recipients must respond by the deadline shown on their letter or risk the disenrollment of their family members from UC benefits.

Biological, adopted, and foster children are not subject to the reverification process. Dependents who have been verified in the last 3 years are also exempt from this process.

This presentation on the Family Member Eligibility Verification (FMEV) for benefits was part of the 'How to' series, covering its history, purpose, and key procedures. Tess outlined essential details, including the verification process implemented by UC in 2015. Attendees learned about contacting UnifyHR for questions, data security measures, and the list of eligible family members. The re-verification process, which started in February and continues through January of 2026, was discussed, along with the various communication methods used to inform employees and retirees. A demonstration of the UnifyHR portal showcased document submission steps, emphasizing early submission to prevent issues. Lastly, Tess provided guidance on obtaining necessary records and encouraged attendees to review their benefit enrollments in UCPath, concluding with a Q&A session and support reminders.
